• Follow-up Site Questionnaire (PFU02)

Sites with IRB/REB approval for the October 2024 Amendment should:

• Request an online survey link: CKiD Continued Follow-up Request Form
  • Survey link will be provided for participants to complete the 01/01/25 version of the online survey (WFU01)
• Alternatively, sites may temporarily print and administer the 01/01/25 version of the WFU01 form.
• Under the October 2024 Amendment, the Phone/In-Person Follow-up Interview Form (PFU01) will no longer be used because it collects the same data as the online survey (WFU01).

Sites WITHOUT IRB/REB approval for the October 2024 Amendment should administer the 09/15/21 version of PFU01
