Feedback and Reports for Families

Below are patient-specific study results that are available to share with participants and their families after a CKiD study visit.

Kidney Function and Other Lab Results

Access on Nephron Report Menu using your Nephron username/password.
Results include (but not limited to):
- GFR Results (including serum creatinine and cystatin C)
- Renal Panel (including urine protein and urine creatinine)
- Lipid Panel

Use the Lab Result Feedback Letter Template to create a feedback letter to share lab results with families.

CPET Feedback Letter and Script

Use CPET Feedback Online Generator to create an individualized participant feedback letter and accompanying CPET feedback script.

To generate the CPET Participant Feedback letter, sites MUST have a completed CPET Case Report Form (CPET CRF).
After CPET results have been reviewed and documented on the CPET CRF, key CPET values highlighted in the CPET CRF are entered into the CPET online form.
Once the required data is entered, the system generates a standardized, editable feedback letter along with a script to support consistent communication of results with families.
The CPET online generator helps ensure consistency in the presentation of CPET findings across study sites while providing clear and understandable language for participants. The accompanying feedback script should be used to guide verbal discussions and facilitate communication of results.

Cardiac CT Feedback Letter

After receiving the Cardiac CT research report, sites may use the Cardiac CT Results Letter Template to share the participant’s calcium score and any additional findings with the participant and family.
Review the Cardiac CT Results Letter Instructions for guidance on which data elements from the research report should be entered into the letter.
